In consideration of being permitted to enter [VENUE NAME] (“the Venue”), located at [ADDRESS], and to allow my dog(s) to use its off-leash areas, bar, and premises, I, the undersigned (“Guest”), agree to the following on behalf of myself, my dog(s), my guests, and my heirs and assigns:
1. Assumption of risk
I understand that entering an off-leash dog environment where alcohol is served involves inherent and unavoidable risks, and I voluntarily assume all of them, including without limitation: dog bites, scratches, jumping, knock-downs, and dog fights; unpredictable behavior of any dog regardless of training or history; exposure to dogs that may be unvaccinated or carrying communicable disease (including parvovirus, distemper, kennel cough, or rabies); slips, trips, and falls on wet, uneven, or turf surfaces; contact with water features; insects, allergens, and wildlife; and injury arising from my own or others consumption of alcohol.
2. Dog behavior
I acknowledge that any dog, regardless of its usual temperament, may act unpredictably. I represent that my dog(s) is currently vaccinated as required by the Venue, is in good health, is not sick or in heat, and has no history of aggressive behavior or biting toward people or animals.
3. Alcohol acknowledgment
I understand alcohol is served on the premises. I agree to drink responsibly, to remain in control of my dog(s) at all times, and I acknowledge that I am solely responsible for supervising my dog(s) regardless of my level of alcohol consumption. I will not allow my dog(s) to consume alcohol or any food or drink not intended for dogs.
4. Responsibility for my dog
I am solely responsible for the behavior of my dog(s) and for any injury or damage they cause to any person, animal, or property. I agree to keep my dog(s) under direct supervision and control and to immediately leash and remove them if directed by staff.
5. Release and waiver
I release, waive, and discharge the Venue, its owners, operators, employees, agents, and affiliates (“Released Parties”) from any and all claims, liabilities, or causes of action for any injury, illness, death, or property damage to me, my dog(s), or my guests arising out of or related to my presence at or use of the Venue, whether caused by negligence of the Released Parties or otherwise, to the fullest extent permitted by law.
6. Indemnification
I agree to indemnify and hold harmless the Released Parties from any claim, loss, or expense (including reasonable attorneys fees) arising from my or my dog(s) conduct on the premises.
7. Medical treatment
I authorize the Venue to secure emergency medical or veterinary treatment on my behalf if I am unable to do so, and I accept financial responsibility for that treatment.
8. Rules
I acknowledge that I have received, read, and agree to abide by the Venue’s posted Rules & Code of Conduct, which are incorporated into this agreement by reference.
9. Governing law & severability
This agreement is governed by the laws of the State of [STATE]. If any provision is found unenforceable, the remaining provisions remain in full force. I have read this waiver, understand it, and sign it freely.

Questions operators ask
Are dog park bar liability waivers actually enforceable?
Often yes, but it depends on your state. Many states enforce well-drafted waivers for ordinary negligence, while few enforce waivers for gross negligence or willful misconduct. Clear assumption-of-risk language and a signature or digital acceptance strengthen enforceability. Have a local attorney confirm.
Why does the waiver need to mention alcohol?
Because serving alcohol around off-leash dogs adds risk that a generic dog-park waiver does not address. Calling it out shows the guest knowingly assumed that specific risk and agreed to supervise their dog regardless of how much they drink.
Do guests need to sign a new waiver every visit?
Not necessarily. Many venues capture the waiver once at membership sign-up and re-confirm it annually or when terms change. A per-visit digital acknowledgment at check-in is the most defensible approach.
Does a waiver stop a dog-bite lawsuit?
It reduces exposure but is not a force field. Most states impose some owner liability for dog bites regardless of a waiver, and a waiver cannot sign away the rights of other guests. Carry general and liquor liability insurance alongside your waiver.
